Medicare Introduces Coverage for Obesity Drugs, Expanding Access for Millions of Older Americans

Starting July 1, 2026, Medicare will provide coverage for GLP-1 medications used to treat obesity, allowing eligible beneficiaries to access these drugs for a copay of $50 per month. This initiative, part of the Bridge demonstration program, is a groundbreaking move as it temporarily circumvents federal laws that previously restricted coverage for obesity treatments.

With over 69 million Medicare beneficiaries, and estimates suggesting that 15 to 20 million older adults may qualify for these drugs, the program could significantly increase market access for Novo Nordisk and Eli Lilly. However, the rollout may face challenges, including a cumbersome prior authorization process and potential strains on healthcare providers due to increased demand.

Moreover, the coverage is set to expire at the end of 2027 unless extended or made permanent, creating uncertainty for patients who may need long-term treatment. The $50 copay is a substantial reduction compared to out-of-pocket costs, but it does not count towards the Part D deductible or the annual out-of-pocket cap, which could still pose affordability issues for many beneficiaries.

The eligibility criteria for the program are broader than those typically seen in commercial insurance, allowing more patients to access necessary treatments. While this initiative is seen as a positive step towards improving healthcare access for older Americans, experts caution that the healthcare system may struggle to accommodate the anticipated influx of patients seeking these medications
